New Vaccine Information:
All students entering 7th and 12th grade will need to be vaccinated against meningitis; a single dose of meningococcal vaccine should be administered at age 11 or 12, and a booster shot at 16 or older.

 

Students who will be in 6th grade must provide proof of the TDAP Vaccine to the Middle school Health Office by end of September of that school year to prevent exclusion from school.. 

Medication (Over the Counter or Prescription) CANNOT be given without a note from the doctor.

Physical/Re-certification forms must clearly state the date of the physical exam, clearance for sports and be signed by the physician. Some doctor forms are not adequate, please use the one attached to the web page to ensure clearance. All sports REQUIRE a re-certification form and physical form be turned in prior to EACH sport season.

OUTDATED, UNSIGNED OR MISSING FORMS PREVENT YOUR STUDENT FROM TRYING OUT.

 

All 7th Grade Students need a State mandated physical on file in the Health Office.
