Student Withdraw / Address Change Form

If you are withdrawing your student from the Gananda CSD, the Change of Status Form must be completed and submitted to the main office of the school your child(ren) attends.

If you are moving within the Gananda CSD, you need to complete the Change of Status Form indicating your new address and submit to the main office of the school your child(ren) attends along with a proof of residency in the form of a recent gas & electric bill in your name or house closing papers or lease agreement.
